COVID 19 Vaccination Self-Certification

In the interest of maintaining the health and safety of students, employees, guest and all members of our campus communities, the California State University is requiring that faculty, staff and students who are accessing campus facilities at any University location to be immunized against SARS-CoV-2, the virus that causes COVID-19. Students may seek an exemption based on medical or religious grounds.

This requirement is effective at the beginning of the Fall 2021 term.

In this article we will be showing you how to submit and edit your COVID-19 Vaccination Self-Certification.

  1. Open your Web-browser and go to my.fresnostate.edu then click Sign In.

     
  2. Once you have logged in please click on Student Self Service tile.

  3. After clicking on the Student Self Service tile you will then see the form below. This is the Covid-19 Vaccination Self Certification form that must be filled out completely before you will be able to access your Student Self Service portal.

     
  4. Please select one of the four options located in Section I.

    • If you select "I have received the approved vaccination" option, then you will be prompted to provide additional information.
      Additional Infomation
      Fill this out to the best of your ability. 
      Note: If you have a 2 dose vaccine the second dose date is required.  If you have only recieved one does, come back to this form and fill it out correctly after receiving your 2nd dose. You will not be able to access your Student Center until both doses have been received and recorded here (currently waving the 2 week waiting period after the final dose). 
       
  5.  Click the box in Section II to Certify that the information you have provided is correct.

  6. a) If in Section I you selected the "I have received the approved vaccination" you must attach a copy of your COVID-19 Vaccination Record Card in Section III. 

    b) If in Section I you selected the "I have received a Medical Exemption" you must attach a copy of your Medical Exemption in Section III.

     
  7. When the form has been completed, click the "Submit" button.

     
  8. You will see a Confirmation Page. 

 To view or edit your submitted Vaccination Self-Certification you may click on the "COVID Vaccination Self-Certify" link.
